Pre-Application Forms for St. Margaret’s House

Here is the pre-application form which will serve as the entry to the September 7 lottery.  Note that this is a 2-page form.  

There are several ways to enter a pre-application into the lottery:

• In person

• Via mail

• Online at www.smhnyc.org

The Russian and Spanish versions are for information purposes ONLY. Pre-applications not completed in English will be disqualified.
